Using subjective complaint statuses to emulate a dictating doctor The paragraph symbol (¶) indicates that a particular status has "extended" verbiage associated with it.  The "extended" verbiage additionally includes the radicular sensory sets on the Related ¶ tab and the aggravating/relieving factors on the Worse/Better ¶ tab. One initial visits and comprehensive exam visits, a user will typically choose statuses that are comprehensive to generate rich verbiage comment for that visit.  A special status of Routine and Initial ¶ is used for initial visits so that the verbiage doesn't say that "the patient returned for today's visit" or similar.  Routine and Initial ¶ is the generic status used for initial visits, but also sometimes preferred by doctors choosing rich verbiage content in personal injury cases. Extended verbiage statuses are: Routine & Initial ¶ New Problem ¶ Improved ¶ Worse ¶ Unchanged ¶ Recurrence ¶ Exacerbation ¶   Concise verbiage statuses are: Improved Slightly Improved Much Improved Worsening Slightly Worse Much Worse Unchanged Recovered No SmarText – this status is used to keep the complaint's findings as part of the patient's record, but turn off the verbiage until the complaint recurs or is exacerbated and needs to become part of the generated note or report.

Using statuses to emulate a dictating doctor.  Use extended (¶) statuses for rich verbiage content, such as for initial visits and re-exam intervals.  Use concise statuses to generate concise verbiage for follow up visits. Notice similar statuses such as Improved and Improved ¶.  These statuses both report that a complaint is improved, but the Improved ¶ status additionally includes the radicular sensations from the  Related ¶ tab and the aggravating/relieving factors from the Worse/Better ¶ tab.

Subjective Complaint Customization

From within MyEMR for Windows, go to Tools->Customize->Subjective->Complaints to customize, add or modify complaints and to associate them with body regions.

Customizing aggravating & relieving factors

 To customize subjective verbiage, choose:

    Tools->Customize->Subjective-> Complaints

from the main frame menu of MyEMR for Windows.  The following figure will appear.  Here you will see a tree view named "Complaints on File".  There are 22 general body region categories.  In each category, you will see lists of complaints typically associated with each region. 

To customize aggravating and relieving factors, select a complaint in the tree view, and then press either the Aggravations or Reliefs button to see a window as indicated in the second figure below.

 

1.    To associate aggravating (or relieving) factors to the current complaint, choose it in the left box and press the ">>" button to move it into the selected box.

2.    To sort the order that they appear in the right box, drag and drop them to the desired order.

3.    To add a new aggravating or relieving factory into the "Available" list on the left, you can right-click to get a context menu to add a new item.  The left list is your master catalog of aggravating or relieving factors.  The right list represents the aggravating or relieving factors associated with the currently selected complaint.
